How Filler Dissolving Works

What Is Filler Dissolving?

Hyaluronidase is an enzyme that breaks down hyaluronic acid. In aesthetic medicine it can be used, following medical assessment and prescribing, to dissolve cross-linked HA dermal filler. It is sometimes referred to by the brand name Hyalase.
Correction Does Not Always Mean Dissolving Everything
Some people want all of their HA filler removed. Others only want a small area refined, a migrated edge softened or an uneven result brought back into balance. We do not assume that every concern needs a full dissolve.
Where appropriate, a targeted correction can preserve the parts of the result you still like while addressing the area that is bothering you.

What Can and Cannot Be Dissolved?

Hyaluronidase is used for HA dermal filler. It does not dissolve every injectable product. Non-HA and collagen-stimulating fillers such as Radiesse and Ellansé are not broken down by hyaluronidase, and permanent fillers need a different assessment and management plan.

Planned Filler Correction

A planned appointment is suitable when the result has settled but you are unhappy with the volume, shape, balance, texture or placement. There is time to discuss whether a small targeted dissolve, a broader correction or no dissolving at all is the best route.

Consultation and Treatment Process

What Happens at rtwskin?

A considered consultation. We listen to what you dislike, examine the area and discuss the result you would prefer. The plan is shaped around your face, not around a standard amount of dissolving.
Your filler history. We review when and where the filler was placed and, where known, which product was used. This helps distinguish HA filler from products that will not respond to hyaluronidase.
Medical and allergy assessment. We take a detailed medical and allergy history, including previous reactions to hyaluronidase and significant bee or wasp sting reactions. Where clinically appropriate, rtwskin carries out an intradermal Hyalase skin test and assesses the test site before treatment. If the assessment is satisfactory, filler dissolving can usually be carried out during the same appointment, although a skin test cannot completely eliminate the possibility of an allergic reaction.
A proportionate treatment plan. The clinician decides the amount and injection points after examining the area. Treatment may focus on a small pocket of filler or a wider area, depending on the concern.
Treatment and review. Hyaluronidase is injected with a fine needle. Topical anaesthetic may be used where appropriate. A review or further treatment may be recommended after the first response and swelling can be assessed properly.

How Quickly Will I See a Change?

Hyaluronidase begins working straight away, but the appearance immediately after treatment is not the final result. Swelling from the injections, the type and amount of filler, its degree of cross-linking and the area treated can all affect how quickly a change becomes clear.

A meaningful assessment is usually possible from around 48 hours, while fuller settling can take up to two weeks. HA fillers vary in formulation and degree of cross-linking, so some break down more readily than others. Denser or more resistant products, larger volumes and complex migration may need a staged approach rather than one aggressive session.

What Happens to the Filler and Natural Hyaluronic Acid?

The dissolved HA filler does not grow back. Filler dissolving cannot guarantee that the tissues will look exactly as they did before treatment, and hyaluronidase may temporarily reduce naturally occurring HA in the treated area. The area can look flatter or feel less hydrated while it settles, but this effect is temporary and reversible.

When Can I Have Filler Again?

New filler should not be rushed into an area that is still swollen or changing. At rtwskin, the area is allowed to settle and is reviewed before a new treatment plan is agreed. Around two weeks is commonly the minimum, although your clinician may recommend longer depending on the area, response and reason for dissolving.

Aftercare Following Filler Dissolving

You will receive aftercare suited to your treatment. Temporary redness, swelling, tenderness, itching or bruising can occur around the injection sites.

avoid make-up for the first six hours
follow any personalised advice about exercise, heat, alcohol and other treatments

Can rtwskin dissolve filler that was injected at another clinic?

Yes. rtwskin can assess and correct suitable HA filler regardless of where it was placed. Your consultation will focus on the current result, what you would like to change and the most appropriate way forward.

What type of filler can hyaluronidase dissolve?

Hyaluronidase dissolves hyaluronic acid dermal filler. It does not dissolve non-HA products such as Radiesse or Ellansé, and it is not a universal solution for permanent fillers. If the product is unclear, we will assess the area before recommending treatment.

Will all of my filler need to be dissolved?

Not necessarily. Some concerns can be treated with a targeted dissolve, while others need a wider correction. The right approach depends on the placement, amount and type of filler and the result you want.
